I have mine middle of the tank medium flow and I feed it a few times a week

Should I cut away the dead material and mount the healthy eyes separately

I'd say leave it I've see stuff come back from worse, feed those eyes to help them grow back a once a week dip in ReVive probably wouldn't hurt either
