Sorry to hear that Paul, this is one of the risks you take with having a carpet anemone. Its not a matter of the fish swimming into the anemone, it only takes a small brush against it for it to be stung and become a meal.

I cant really see anything. Do a google search for aptasia and majano anemone and see if it looks like any of the images. I would bet it most likely is one of these two.
