My money is on the sandbed. I would imagine that the sand was absorbing nutrients during the time when WC were 1x per month, and with it being disturbed, the nutrients were released into the water. It could be as simple as the pistol getting into a particularly nasty part of the sand bed.

I did this a few years ago, in 2009.
We had our mandarin in a small tank, 30g, and we fed pellets, mysids, cyclopseeze.
We tried Melev's mandarin diner, which our other small fish got into routinely. We fed a combination of live food and frozen food, and we kept our tank heavily stocked...

Do steer away from crushed coral, go for fiji pink & your nutrient levels (long term) will be soooo much better in the nano.
These anems are IMHO the nicest you can find in NJ.
James' mother nem has been in captivity for a very long time (he gave us a split in 2009 from the same RBTA) and they are very hardy, and really gorgeous anems. If you want a Bubble Tip, this is the best you are going to get.
You can keep the pistol & goby in a less-than-deep sand bed, but I do think they need it.
I would go light on the sand, they will be OK in 1".
The goby would probably be OK without the sand, but I would worry about the pistol.
The only other thing you could consider is an itty bitty nano...
I found something similar a few months back, I thought mine was an olive snail. It liked to burrow in the sand bed and it was a carnivore. Mine was about 1/8" when I found it. It lives in the 12g and I see it maybe once every two months.
